You are flying to Poland by plane, fill in the Traveler Location Card

If you cross the border of the Republic of Poland by plane after July 16, 2021, fill in the Traveler Location Card before check-in.

What is the Traveler Location Card for?

The Traveler Location Card is to ensure the safety of you, your relatives and everyone with whom you have contact. In the event that an infected person is aboard the aircraft, this will enable it to reach you and other passengers quickly, helping to reduce the spread of the virus.

Fill in the Card before travel, during the journey or at the latest before check-in

KLP is filled by everyone who travels by plane and intends to cross the border of the Republic of Poland.

What do you need to prepare?

You will need:

  • country where the journey began
  • flight number,
  • date of arrival,
  • first name and last name,
  • PESEL number or identity document (ID card or passport),
  • contact details (phone number, e-mail address),
  • addresses of all places where you intend to stay in Poland for the first 14 days (e.g. hotel, family home, holiday resort),
  • information whether you have a certificate that you are convalescent or fully vaccinated or whether you have a negative PCR or antigen test for SARS-CoV-2 virus.

What do you have to do?

  • Click Fill in the form.
  • Pass it e-mail adress. Click Confirm Email Address.
  • You will receive an email with a link to the form. Click on the link.
  • Fill in all the mandatory fields in the form (details can be found in section What you need to prepare).
  • Accept all consents and declarations.
  • Send your application. You will receive a confirmation to the e-mail address provided in the form.
  • You can print the confirmation or present it to the airport staff on a mobile device (e.g. phone, tablet).

Can you submit an application on someone else's behalf?

You can fill out one Card on behalf of other people who travel with you. You can enter the data of several people on one Card.

Attention! You can complete one KLP for several people only if they will be in the same places as you. For example, if you are traveling with your family, but your children will be staying at a different location, you must complete a separate Form for them.

The card is sent to the Sanepid (State Sanitary Inspection). You must show the confirmation of sending the Card at the airport in Poland during check-in (in the form of a printout or a pdf file on a mobile device). You will receive confirmation by e-mail

What is the legal basis?

Ordinance of the Council of Ministers of July 9, 2021 amending the ordinance on the establishment of certain restrictions, orders and bans in connection with an epidemic (Journal of Laws, item 1262).
